When one receives a life-limiting diagnosis, it can send an individual and their family into a tailspin of emotions including shock, pain, fear, anger, confusion, isolation, denial and grief.

It is critical that those who are facing death as well as their loved ones have somewhere to turn for help.  The unfortunate truth, right now, is that many people are dying without the support they need and deserve.  Heart House is dedicated to making it easier for as many people as possible.

Your gift will ensure that compassionate hospice care remains available at no cost to your friends, family, and neighbours when then need it most.  Donations are directed into the areas of greatest need at the Hospice, supporting programs and services for 1,000 individuals and 2,500 loved ones per year.  

With many innovative and holistic wellness programs, social workers, spiritual care providers, health and wellness coordinator, engagement counsellor, children and youth counsellors, bereavement counsellors and 287 active volunteers, you can be assured that your donation will be put to good use.  All gifts to the Hospice are used locally in Mississauga and Brampton.  Your gift will be acknowledged in our annual report (unless you inform us that you would not wish to be publically recognized) and you will be invited to a special gathering of Hospice donors.

Your Gift

For monthly donors signing up for monthly withdrawals from your bank account, please read and sign the following PAD agreement and attach a blank cheque with “VOID” written on it and return with this form. Authorization for Automatic Monthly Withdrawals from my Bank Account:

The debit will be processed to my/our account on the 15th day of each month, or the next business day. I may revoke my authorization at any time, subject to providing notice of 30 days. To obtain a sample cancellation form, or for more information on my right to cancel a PAD Agreement, I may contact my financial institution or visit Canadian Payments Association. I have certain recourse rights if any debit does not comply with this agreement. For example, I have the right to receive reimbursement for any debit that is not authorized or is not consistent with the PAD Agreement. All gifts at a value greater than $25 will receive an official tax receipt. Heart House Hospice is committed to protecting our donor and client information. This information is collected for purposes of processing your donation and keeping you informed of our programs, services and fundraising. We do not rent, trade, share or sell our information.
