In-home support
As a community hospice we understand there is no place like home and that support in your home is often what you and your family need. In order to access our in home services, one of our Heart House Hospice staff needs to complete a visit, an assessment and a referral. Of course, we will come to your home to do that!
We have a number of in home programs you will be assessed and referred to including:

HOSPICE COUNSELLING
Our hospice counsellors come to your home to provide counselling, information, education and advocacy to you and your family. They provide emotional support and can help you and your loved ones get your questions answered.
SPIRITUAL CARE
Opening up possibilities for hope during major life events is the foundation of spiritual care. We have specially trained staff and volunteers to visit you in your home and support your spiritual care needs.
IN HOME VOLUNTEER VISITING
Volunteers will come to your home to spend some time doing what you would like to do. Our volunteers are there to share and spend time with you; reminiscing, talking, reading, writing cards, listen to music or providing company.
IN HOME COMPLEMENTARY THERAPY VOLUNTEERS
Complementary Therapies are effective in eliciting a relaxation response, reducing anxiety and changing the perception of pain. Our specially trained volunteer will come in home and provide treatments for you and your loved one.
IN HOME CHILDRENS SUPPORT
Living with serious illness or a death is difficult for children and families. Our HUUG counsellors will come to you home and provide support to your family. Our counsellors will help children; understand grief, express thoughts, feelings, and emotions, remember loved ones who have died, communicate and support one another and develop tools to cope with grief in healthy ways.
BEREAVEMENT SUPPORT
While we encourage people to visit us in the office for bereavement counselling we understand there may be times and reasons that this is not possible. In some circumstances our specially trained bereavement staff and volunteers will come to your home to provide grief support.
