What do we offer?
Heart House makes a meaningful difference to someones end-of-life experience.
We are a team of professional, highly-trained, caring volunteers and staff who become
devoted, compassionate, supportive companions to our clients. We provide support
and comfort.
We are there to support you during a very emotionally difficult time in your life.
We are there to listen and provide comfort.
We advocate for, and attend to the non-medical, spiritual, emotional and respite
needs of our clients and the people who love them.
We provide service that is adaptable and responsive to the special needs of the
individual and their loved ones, and is not disease specific.
We will assist you to navigate the often complex health system.
Who do we serve?
Heart House provides support for community members and their families living with
a life-threatening or terminal illness whose prognosis is less than a year.
We provide services in the Cities of Mississauga and Brampton.
How do you access our services?
Anyone with the permission of the client or family members can make a referral to
Heart House. A referral can be made by telephone (905-712-8119), fax (905-712-4029)
or email
ckee@hearthousehospice.com.
The information we require is the name, address and telephone of the person or family
you are referring. You will also be asked about diagnosis and prognosis (if known).
A Hospice Care Coordinator will follow up with the client and family and may suggest
programs to meet their immediate needs. A home visit will be scheduled at which
time the Hospice Care Coordinator will describe the services and programs available
at Heart House. If we are unable to meet their needs, we will help them to access
other services and programs that may be more appropriate for their needs.
How much does Hospice care cost?
A fee would prevent many people from receiving the amount of help they need, therefore
all of our services are provided
free of charge to the client and their families.
Donations are always welcome.
